- Download and fill out the Retiree Cane Card Request Form and provide it to their direct supervisor on or before their last date of employment.
- Have your direct supervisor sign the form.
- Take the completed form and your existing Cane Card to the Cane Card Office, located in the McKnight Building, 1st FL (5807 Ponce De Leon Blvd) between the hours of 8:30 am and 5 pm, Monday through Friday. (Note: If you do not have your existing Cane Card, you will be charged a lost Cane Card fee.)
- The Cane Card Office will receive the existing Cane Card and completed form, and provide the new Cane Card to the employee for use on campus.
